Registration Information:
The tournament dates are October 11-15, 2011. To enter the tournament a check or money order in the amount of $5,000 must accompany this form. This fee covers up to six anglers in the base tournament categories.

• Personal Checks: Must be drawn on a U.S. Bank, Payable in U.S. Dollars and received by 9/24/10
• Money Order and Cashiers Check in U.S. Dollars and received by 10/03/11.
Payments On Site:
• Money Order, Cashiers Check or Travelers Check: U.S. Dollars drawn on U.S. bank Only.
• No personal checks or cash can be accepted on site.
